　　Overview

　　Gentiva Home Hospice is looking for a Registered Nurse (RN) to join our team in Branford, CT.

　　This position is PRN/Per Diem status

　　Office Hours of Operation:

　　Monday - Friday, Days (Example: 8 AM - 5 PM)

　　Flexibility to meet patient and business needs

　　With occassional "On Call"

　　Drive to patient home or facility

　　Valid Driver's License

　　Active Car Insurance

　　Mileage Reimbursement

　　Reports directly to the Administrator, Executive Director or Patient Care Manager Senior and willmaintain primary control and professional management of each patient. You will act as primary liaison between physician, patient/family and hospice team.

　　Provide direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable case load

　　Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care

　　evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient

　　Maintain all assigned patient clinical records

　　Participate in company quality assessment and performance improvement programs

　　About You

　　Compassionate, kind and empathetic to your patients and their families

　　A current RN license (in the state of requested employment or ability to obtain)

　　1+ years of nursing experience in hospice, home health, or oncology

　　Compensation may vary within the rate/salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

　　Rate Range – Min $ 32.32 to Max $ 48.49 (hourly)
