We are currently seeking aRN Hospice Case Managerto provide caring, compassionate andpatient-centered care to hospice patients in the Milwaukee County As aRN - Hospice Case Manager you will: Serve as the clinical lead for ateam of professionals whose goal it is to care for individuals on theirfinal journey in life. Work with your patients and their families inconjunction with the interdisciplinary team to establish goals for theirfinal days. Establish a plan of care that reflects assessment findingsto the meet patient needs. Case conference with team members weekly andas necessary to discuss progress in reaching the goals you and thepatient have established.
