Responsibilities Job Summary TheRegistered Nurse in Home and Community Based Services provides anddirects provisions of nursing and personal care to patients in theirhomes as prescribed by the physician or as authorized by thestate/program and in compliance with applicable laws, regulations, andagency policies. The Registered Nurse coordinates the plan of care withother health care professionals involved in care and helps to achieveand maintain continuity of patient care by planning and exchanginginformation with physician, agency personnel, patient, family, casemanager and other community resources. Specific JobDuties/Responsibilities Provides high quality clinical services withinthe scope of practice and within infection control standards, inaccordance with the plan of care/service plan, and in coordination withother members of the patient/client\'s care team from admit throughdischarge. Completes clinical nursing assessments in accordance withfederal and/or state program requirements and as required by payerEnsures the patient/client\'s eligibility and medical necessity forservices as defined by payer source and agency policy. Develops andrevises individualized plans of care and/or service plans according tofederal and/or state program requirements with other communityproviders. Ensures plan of care frequency and duration meets the needsof the patient and initiates plan of care revisions as needed withphysician approval. Education and Experience Experience Desired Aminimum of one year experience as an RN preferred. License RequirementsMust have current RN licensure in state of practice. Current driverslicense, vehicle insurance and access to a dependable vehicle or publictransportation. Current CPR certification required.
