Job Description: The Oncology Nurse Navigator is accountablefor the clinical care coordination of patients. They provide directpatient care and comprehensive coordination for patients and theirfamilies to proactively identify, and resolve needs by eliminatingbarriers to timely care, facilitating flow through system, and ensuringall patients have immediate access to emotional support and clinicalguidance. The Oncology Nurse Navigator works in collaboration with thecare team and other multidisciplinary teams to establish and coordinatethe plan of care. The Oncology Nurse Navigator will focus on caring andguiding oncology patients from their time of diagnosis through theircancer journey.
