　　Job Summary

　　Responsible for coordination of an interdisciplinary team approach in the provision of skilled nursing assessments, planning and care in order to maximize the comfort and health of patients and families consistent with the policies and procedures of Lourdes Homecare/Hospice. Develops a holistic multidisciplinary plan of care. Interacts with the care management team and physicians to determine appropriate home care/ hospice service for patients/clients based on regulatory guidelines for each line of service.

　　Minimum Required Qualifications

　　Education: Graduate of accredited school of nursing

　　Licensure:Licensed as a registered nurse in KY. Current BLS certification required

　　Certification(s):Willing to obtain CHPN certification after 2 years experience

　　Experience: two years of acute nursing experience medical surgical, OB, psychiatric, pediatrics etc.
