Ambulatory RN Care Manager provides care management services and community resource coordination to patients and their families who utilize inpatient and outpatient medical services. This position is part of a centralized care coordination model for the Innovative Health Alliance of New York (IHANY), serving attributed lives, in both their primary and specialty practices. This position will be based out of the St Peter’s Health Partners Medical Associates care coordination department. While based out of Ellis Medical Group, this position will affect change by performing community assessments, working collaboratively with the providers and other interdisciplinary team members to serve our most vulnerable lives both in the community and as they transition through the healthcare continuum. The work involves the use of motivational interviewing, assessment and clinical interventions aimed at positively affecting the physical, cognitive, and emotional functioning of patients and their families. The RN Care Managers actively participate in QI initiatives within the ambulatory setting. The RN Care Manager plays a significant role within our Accountable Care Organizations and assists members in our accountable care organization where we are responsible for ensuring the highest outcomes, satisfaction and value in the total cost of care for our most vulnerable community members. Responsibilities also include collaboration with the multi-disciplinary team and delivering Integrated Care, quality outcomes and ensuring appropriate utilization of resources.
What you will need:
BSN required (NYS Registered Professional Nurse licensure). AD will be grandfathered and must meet performance standards. Previous experience required in care management at least one of the following settings: disease management, home health care, hospital or physician office setting. Preferred Certification in Case Management (CCM).
Experience Requirements:
5 years of RN experience in Care Management required. Prior electronic medical record (EMR) experience required. Previous project leadership experience required. Possess strong community resource knowledge. General knowledge of insurance plans and clinical resource management
Excellent oral and written communication skills. Excellent customer service skills. Ability to work independently, setting priorities to coordinate care plan efficiently. Proven leadership skills and ability to work effectively in a team environment. Highly organized and detail-oriented, ability to perform multiple tasks. Effective behavioral and educational strategies, including, but not limited to, motivational interviewing, teach-back method and self-management support. Recognize the role of cultural, social, economic and behavioral factors in accessibility, availability, acceptability, and delivery of public health services, and respond to diverse needs that are a result of these differences. Incorporate strategies for interacting with persons from diverse backgrounds (e.g. cultural, socioeconomic, educational, racial, gender, age, ethnic, and affiliation, mental and physical capabilities).
Pay Range:$31.50 - $44.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
