Job Summary and Qualifications
Performs inpatient admission assessment for adult and adolescent patients. Responsible for assessment, initiating individualized care plan and developing medication reconciliation.
Performs behavioral, chemical dependency, current and historical medical evaluation Admission Assessment
Develops and complies individualized Plan of Care based on patient-centered model and standardized criteria sets.
Collaborates with physicians, nursing staff, internal medicine and dietician.
Provides resource information for all staff.
Referrals for specific medical conditions requiring dietary consultation
Performs Data Collect role through documentation of select demographics and approved queries in Physicians Patient Keeper module
Adheres to hospital policies and procedures.
Demonstrates understanding of and adherence to the Code of Conduct.
Conduct reflects the Company’s values and a commitment to the Code of Conduct.
Performs other duties as requested or required.
What qualifications you will need:
Must have a minimum of an Associate’s Degree in Nursing and a current Texas State RN license or compact license is accepted.
Current BLS with American Heart Association or American Red Cross and SAMA certification or attainment during the orientation period within 30 days of hire.
Experience or education with a focus on crisis stabilization, age specific needs, and/or knowledge of chronic mental illness and chemical dependency is preferred.
Strong communication skills in both oral and written format recommended.
Proficient in [HCIS] system.
Typing strongly preferred with a minimum of 20 words per minute.
Must be flexible and service oriented.
FUNCTIONAL CAPACITY AND WORKING CONDITIONS:
Extended walking and standing daily. Frequently carrying up to 40lbs. Must have ability to grasp. Requires corrected vision and hearing to normal range. Must have full range of motion to include restraining patients. Some exposure to communicable diseases or bodily fluids, hand-eye coordination, manual and finger dexterity. OSHA BLOODBORNE CATEGORY I.
