  COLORADO STATE VETERANS COMMUNITY LIVING CENTER – HOMELAKE:

  To ensure a safe, sanitary, well-maintained, comfortable living and working environment for residents of a 60-bed skilled nursing home and 50-unit domiciliary, staff, and visitors. The facility has embraced the Culture Change and Eden concepts. We strive to provide exemplary service to the residents by fulfilling daily and special requests that the residents may have. Exemplary care is given in cleaning and sanitizing and in keeping the resident rooms and their atmosphere clean, organized, homelike, and safe. We need to provide the residents with the kind of service that they need and deserve.

  Description of Job

  This is a temporary, full-time position within a Veterans Community Living Center - RN license is required from the Colorado Department of Regulatory Agencies or any state that participates in the eNLC to fulfill the duties of this position. Shifts: Day, Evening, or Night shifts to include holidays and weekends, as needed.

  Please note: These working arrangements are subject to change.

  Primary Job Duties:

  Resident Care/Safety:

  Assess, plan implement, evaluate, and provide direct care to residents in a skilled nursing care unit and/or a domiciliary unit.

  Coordinate duties as appropriate for specific disciplines and monitor work progress and completion i.e.: RN, LPN, C.N.A.

  Monitor the resident’s condition and be attuned to any change of condition.

  Monitor the safety of residents and act to resolve problems and notify the MD and team of care needs.

  Documentation/Admissions/Other:

  Complete documentation in residents’ charts and provide continuity of care through this documentation.

  Administers medications and treatments per MD orders.

  Meets the psychosocial needs of the residents by direct contact and counseling.

  Assess and meet the functional needs of residents with nurse assessments and planning.

  Facilitates medical appointments and assists in coordinating medical transports.

  Assign duties as necessary to meet these. Documents residents’ healthcare status completes forms and documents the resident’s status.

  Completes all necessary agency documents and maintains the electronic medical record.

  Provides continuity of care through documentation.

  Coordinates care with other nursing staff.

  Completes admissions, acts as a resident advocate, educates residents and significant others, and documents teaching. Interacts with other health care professionals (MD, OT, PT, SW, etc.) to ensure resident health care needs are met.

  Transcribes MD orders, and completes required forms.

  Contributes to the care plan process with the inter-disciplinary team on the care needs of the residents.

  Acts as resident advocate.

  Educates residents on health care concerns/changes in care plan. Etc.

  Coaches/educates staff on health care issues.

  Attend educational workshops and in-services to maintain and update knowledge of changing procedures, methods, policies/procedures, laws, and regulations relating to nursing practice.

  Other Duties as Assigned:

  Responsible for the performance of assigned tasks and duties within the scope of practice for this position that does not fall within the realm of duty statements listed above.Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

  Minimum Qualifications:

  Valid Registered Nurse License from the State of Colorado or any state participating in the Enhanced Nurse Licensure Compact (eNLC).AND

  Two (2) years of professional experience as a registered nurse.Substitutions:

  There is no substitution for the required license.

  A bachelor's degree from an accredited college or university in nursing may substitute for one year of the required experience.

  A master's degree or doctorate from an accredited college or university in nursing may substitute for the required experience.

  If you are using college/university coursework as a substitution for experience, you must attach unofficial transcripts showing the coursework to the application.

  Special Qualifications:

  Must possess a valid/current Colorado nursing license, IV certification (optional)Preferred qualifications: Experience in long-term care or assisted living facility; experience with staff development

  Conditions of employment:

  Must possess and maintain a valid US driver's License.

  A pre-employment full criminal background check will be conducted as part of the selection process as this position is deemed to have direct contact with vulnerable persons. This will include CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks (Skill Survey), Professional License verification (licensure requirements), Drug Screen, CAPS (direct contact with adults – Mental Health Institutes, Regional

  Centers, Veterans Community Living Centers).

  Shift Work: Day, Evening, or Night shifts to include holidays and weekends as needed.

  May be required to travel outside the San Luis Valley for ongoing education (training) and resident transports.

  Ongoing Functional Capacity Testing: Annual RN competency.

  Must possess and maintain a current and valid Colorado RN license obtained from the Colorado Department of Regulatory Agencies or any state participating in the eNLC (Nursing License Compact); IV Certification (optional).

  CPR certification is required.

  This position is considered "essential service" and you will be required to report without delay or interruption to provide essential or emergency services to ensure health, safety, and welfare.

  Ability to handle invasive medical devices and/or equipment.

  Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and explain as to why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination, or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
