Home Town is a growing Home Health and Hospice agency that creates apositive atmosphere for our staff. Home health allows clinicians to slowdown, care for patients one-on-one, treat, heal, and educate patientsand families in the safety of their own home. Duties include:
Directs nursing and home healthcare as prescribed by the attendingphysician
Reviews all plans of treatments for accuracy, appropriateness andcompleteness
Maintains accurate records of all services rendered to patients
Evaluates and regularly reevaluates the therapy and nursing needs ofeach patient
Performs initial admission assessment and evaluation of the Agencyspatients
Ability to assess adequately and reassess pain
Utilizes appropriate pain management techniques
Educates the patient and family regarding pain management
Performs all aspects of patient care in an environment that optimizespatient safety and reduces the likelihood of medical/health care errors
Reports to the physician the patients response to treatments, includingdrugs, and changes in the patients physical or emotional condition
Demonstrates knowledge of medications and their correct administrationbased on age of the patient and patients clinical position
Follows the five (5) medication rights and reduces the potential formedication errors
Teaches, supervises and counsels the patient and family membersregarding the nursing care needs
Performs patient care responsibilities considering needs specific to thestandard of care for patients age
Addresses and supports cultural practices, as long as such practices donot harm others or interfere with the planned course of medical therapy
Supports and carries out the Agencys policies
Participates in the Agencys information management and performanceimprovement programs
Assures that monthly case conferences are done on each patient, recordedand filed in patients chart
Completes annual educational requirements
Maintains regulatory requirements
Adheres to dress code, appearance is neat and clean
Represents the organization in a positive and professional manner in thecommunity
