　　Casual/On Call Rate of Pay is $47/hour

　　Additional pay when working evening/night shifts

　　The Registered Nurse (RN) provides for and assists in meeting the TCU transitional care unit residents’ physical, emotional and psycho-social needs while adhering to facility, state and federal guidelines for beautiful and vibrant skilled nursing care center in Wyoming, MN.

　　This nurse positionn includes:

　　Casual/on-call

　　Day, evening, overnight shift

　　Must pick up three shifts a month; one being a weekend shift

　　12 hour shifts

　　Meadows on Fairview TCU is located in the city of Wyoming, MN, steps away from the Fairview Lakes Hospital and a short distance from Forest Lake and North Branch.  We offer a home like atmosphere where you feel like you belong the minute you arrive!  Our 14-bed Transitional Care Unit is small enough to get to know your co-workers and residents yet large enough to provide you with opportunities and room to grow.

　　Responsibilities/Job Description

　　Monitors personal cares provided to assure consistency with each individual plan of care

　　Administers medications and performs treatments per physician orders and facility policies

　　Provides data relevant to the development of the plan of care

　　RN – Assesses plan of care, resident goals and revises, modify plan of care as necessary

　　Provides reports and documentation related to incidents, change of status and cares provided

　　Qualifications

　　Required:

　　Current MN RN license or by start date

　　AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers by start date

　　Preferred:

　　Comfortable using Microsoft windows and electronic charting systems

　　Work experience in a patient support role, LPN or RN

　　Good written and verbal communication skills to interact with residents, families and physicians or providers

　　Ability to work with and have sensitivity to the needs of the geriatric population
