Job Essentials
1. Develops relationships and collaborates with physicians, facilities, and teams to promote process integration, seamless transitions from one program to another, continuity of care, and to avoid duplicative care management services and processes.
2. Manages the care for clinically complex patients admitted to homecare. Meets with the patient or family in a timely manner and conducts an initial assessment using the appropriate federal assessment.
3. Develops/Reviews a patient-centered plan of care, involving the patient or family or caregiver or significant others in the process to develop goals/outcomes.
4. Advocates on behalf of the patient by communicating and collaborating with healthcare providers, payers, physicians, and community-based services, where appropriate, to establish an appropriate and integrated care plan for each patient.
5. Facilitates transitions of care from one healthcare setting to another.
6. Functions as the team leader to ensure effective day-to-day operations and problem solving, for the Home Health or Hospice team. Oversees care team and coordinates patient activities to ensure quality and cost-effective care.
Minimum Qualifications
Bachelor's degree in Nursing (BSN) from an accredited institution
Current RN license in state of practice
Current driver’s license, insured and reliable transportation and an acceptable driving record
Current BLS Certification for healthcare providers
Two years of clinical nursing experience
One year of experience working in Home Health or Hospice setting.
Ability to communicate effectively both verbally and in writing
Preferred Qualifications
Three years of experience working as a case/care manager in a home health or hospice setting
Experience working with third party payers
Knowledge of home- based health services and resources

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$42.70 - $61.93
