TheCommunity Homes program focuses on building a human habitat whereresidents and staff are living rich and meaningful lives. Nurses arecore members of the Clinical Support team (CST). Their role on the CSTincludes assessing and monitoring the clinical status of residents anddelivering evidence-based clinical care, both directly and through theUniversal Worker and other CST members. The goals are to center servicesaround the resident, to foster resident independence to their highestfunctional capacity, and maximize efficient and cost-effective use ofhealth care resources. The RN is aligned under the Community LivingCenter within the Rehabilitation Extended and Community Care (RECC)Division and is responsible for providing the clinical leadership andexpertise in the Community Homes. He/she possesses intimate knowledge ofLong Term Care federal regulations, VHA guidelines, medical reviewcriteria, Minimum Data Set (MDS), clinical documentation requirements,current standards of care, and compliance guidelines. Registered Nurseduties include the following: a) The RN possesses the knowledge andskills necessary to effectively apply all aspects of the nursing processwithin the collaborative, interdisciplinary practice setting of theCommunity Homes. This broad based clinical knowledge enables the RN toeffectively implement health promotion and prevention interventions,manage acute and chronic illness states, assist veterans in theattainment of optimal levels of functioning through rehabilitation, andprovide palliative care for the resident and their significant others.b) Obtains an update on each resident from the Universal Worker duringeach shift. c) Visits each elder during each shift to monitor clinicalstatus specific to each individual resident and to determine if theclinical needs of the resident are being. d) Assess each resident forunresolved/untreated pain.
