Description:

　　The RN is responsible for admission of patients for home care services, development of the patient plan of care, coordination with patient's provider and other disciplines for the provision of services, and safe delivery of patient care. 1. Collaborates with the healthcare team to create & implement plan of care 2. Documents all relevant information about patient into EPIC EMR system 3. Creates own schedule according to patients needs 4. Assess payment and responsible for ongoing authorizations 5. Educates/instructs patients on medical maintenance 6. Care/Discharge plans
