　　JOB SUMMARY:

　　The Recovery Aid - Transitions provides direct services to individuals with mental illness and substance abuse issues within a supervised living setting called Transitions. Provides support and instruction as needed and assists treatment team members in implementing individualized recovery plans. Completes household duties to ensure cleanliness, safety, and compliance with licensure regulations.

　　ESSENTIAL FUNCTIONS:

　　Provide individualized and group support and training services to adults meeting criteria for the Transitions program in order to promote recovery and meet the program goal of assisting individuals in transitioning from state hospital care to living independently. Under supervision, this position will provide assistance with independent living skills, social skills, personal hygiene, household skills, cooking, nutrition and meal planning education and assistance, and transportation to recreational groups and activities.Ensure a clean and safe environment by performing such task as fire drills, fire extinguisher checks, house cleaning, grocery shopping, meal planning and preparation.Participate in the activities of a self-directed team, and as needed, in assigned or scheduled meetings and trainings events.Maintain, administer, and document medications in accordance with written physicians orders and regulatory standards, and maintain current certification in the Boards approved medication training program. Observe persons for potential side effects of medications and notify appropriate staff as necessary. Administer basic first aid as needed and maintain current certification in CPR, First Aid, and other trainings required by supervisor.Appropriately intervene in crisis situations and emergencies, making referrals and seeking consultation from other team members as needed.Participate with team members in the formulation of person-centered recovery planning. This may also include developing goals, objectives, plans, and data sheets in order to provide accurate and comprehensive documentation.OTHER DUTIES:

　　Ability to interact with people in a supportive and therapeutic mannerKnowledge of the nature of mental health, mental retardation, and substance abuse disordersGood oral and written communication skillsAbility to assist in developing and implementing service plansSkill in data collectionAbility to work as part of a teamMust be able to work independently and with supervision available at broadly spaced intervalsMinimum Requirements

　　EXPERIENCE/EDUCATION REQUIRED:

　　High school diploma and/or equivalent.

　　Valid Drivers license with a safe driving record.
