  POSITION SUMMARY: Provide individualized supports and skill development to individuals in the community. Promote personally meaningful connections and roles in the community. Assist individuals to gain access to needed behavioral health, medical, social, residential, financial and other services appropriate for the individual. Promote recovery by supporting the individual to facilitate relationships and environments that provide hope, empowerment, choice and opportunities that support people in reaching their potential as individuals and community members. Assist individuals in achieving meaningful, satisfying lives through the development of the skills necessary to live independently, obtain employment, attend college, complete their education and have healthy social lives.DUTIES AND RESPONSIBILITIES:* Assist individuals to develop goals and use action steps in their Recovery Plans through the use of direct skills teaching, modeling, accompanying individuals to appointments, leisure activities such as: religious gatherings, educational or vocational endeavors and other activities of interest to provide support and encouragement, to help the individual achieve their goals and valued community roles* Work directly with the individual to complete assessment tools that identify their strengths, interest and preference for rehabilitation service* Develop strategies to assist the individual in identifying, achieving and maintaining valued roles in the individual's community through the use of Evidence Based Practices, Best Practices and other Psychiatric Rehabilitation practices* Help the individual increase awareness of community resources and identify preferred options for the rehabilitation process through direct and indirect skill teaching, identifying appropriate resource material and appropriate resources in the community* Link individuals to community resources and peer supports to promote community integration by using direct skill building and community integration techniques* Assist individuals in gaining access to needed behavioral health, medical, social, residential, financial and other services appropriate for the individual through the use of goal plans, assessment tools, other recovery oriented tools, direct skill building and community integration techniques* Assist the individual in identifying, developing and utilizing natural supports through the use of goal plans, assessment tools and other recovery oriented tools* Teach coping skills including emotional regulation, relaxation skills and anger management using person medicine, CBT techniques, and assessments to identify possible intervention needed* Educate and provide resources to an individual about mental illness, living with mental health challenges, wellness, and living in recovery through direct skill teaching, identifying appropriate resource material and appropriate resources in the community* Provide skill building along with medication education to support the individual to take their medication as prescribed by the physician and encourage the individual to have an open conversation with their psychiatrist for the individual to live a fulfilling and contributing life* Plan/structure and prompt social and leisure time activities on evenings, weekends, and holidays through the use of goal plans, assessment tools and other recovery oriented tools* Help individuals develop their personal support systems using assessment tools, recovery plans and other appropriate tools* Provide individuals with transportation or accompany them on public/private transportation to appointments, recreational activities, day program etc. as needed while teaching the individual skills to do so independently in the future* Ensure individuals have adequate financial support by securing gainful employme t or applying for entitlements.* Provide support to enable individuals identify vocational strengths and challenges, establish vocational or career goals and plans, and teach them to recognize and target symptoms of mental illness that interfere with work* Collaborate with the person needing housing to learn his or her housing choices and the supports needed through the use of goal plans, assessment tools, other recovery oriented tool, direct skill building and community integration techniques* Locate affordable and safe housing with public and private providers. Develop linkages with community resources regarding the availability of rental units, fair market values, rental subsidies, home ownership programs, and tenant/landlord home ownership responsibilities. Staff may use goal plans, assessment tools, other recovery oriented tool, direct skill building and community integration techniques.* Attend trainings, conferences, and view webinars on recovery, psychiatric rehabilitation, trauma, supported employment practices, practice guidelines, substance abuse and cultural competency. Keep apprised of new research and developments in these areas through journals and other resources. Become knowledgeable of Substance Abuse and Mental Health Services Administration (SAMHSA) Best Practices and emerging practices that will provide services to promote recovery and skill development.* Must complete 12 hours of Psychiatric Rehabilitation Orientation course approved by the Psychiatric Rehabilitation Association no later than one year after date of hire.* Must complete 18 hours of training per year with 12 hours specifically focused on psychiatric rehabilitation or recovery practices or both* Facilitate in crisis management situation as appropriate* Consistently work with other team members in an effort to support a positive amiable atmosphere* Perform other duties as assigned

  IMMEDIATE SUPERVISOR: Psychiatric Rehabilitation SupervisorDIRECT REPORTS: None.CONTACTS: Consumers, tenants, vendors, Elwyn staff, families, outside agency representatives, county representative, visitorsEDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:* Bachelor's degree in human services* Two (2) years behavioral health direct service work experience, one (1) year of which must be in Psychiatric Rehabilitation services* Certified Psychiatric Rehabilitation Practitioner (CPRP) or must receive a credential within two years from the date of hire through United States Psychiatric Rehabilitation Association (USPRA)* Must have and maintain a valid driver's license in your state of residence, with a safe driving record* Demonstrated ability to independently problem solve in a creative manner* Demonstrated awareness of when to seek/solicit help the help or guidance from co-workers and/or management or appropriate resource person* Demonstrated ability to communicate successfully and relate well with tenants, families, staff, vendors, outside agencies, visitors and management* Ability to handle multiple priorities* Ability to exercise sound judgment with demonstrated problem solving and decision making skills* Demonstrated good oral and written communication skills
