PRINCIPAL DUTIES ANDRESPONSIBILITIES: Create a positive and supportive program providingtrauma-informed, client-centered care to Young Families experiencinghomelessness. Engage potentially high-risk clients in individualizedcase management, safety planning and planning for housing, employment,and/or education. Establish positive professional working relationshipswith families and individuals assessed and served in the Program.Provide in-home individualized case management, advocacy, safetyplanning and planning for housing, employment, and/or education toparticipants, including, but not limited to, the following activities:Assist participants in finding permanent housing, work closely withlandlords, meet with participants at least once a week, monitor progressof participants in accordance with tools provided and adjust as needed.Maintain positive relationships with property owners renting to youngfamilies in the TH Program. Provide aftercare outreach and assistance.Provide linkages to community resources, which increase opportunitiesfor stable housing and economic independence. Develop relationships withpotential funding sources, establish and maintain community linkages andcollateral contacts, including area shelters and local housingorganizations Attend applicable meetings with network providers, theNetwork to End Homelessness, and others as necessary Participate incollaborating with other agencies/CoC as applicable to the position andknowledge base. Monitoring program files and other paper work. Completeall required reports and tracking tools for participants, fundingsources and the agency with the Program Director, including but notlimited to; monitoring data entry and reporting requirements. Beavailable to work evenings/weekends, when necessary. Respond toafter-hours work related phone calls. KNOWLEDGE, SKILLS, AND ABILITIESAssociates degree and/or 3 years of experience in homeless servicesrequired.
