CP Unlimited operates an extensive community based living program, day habilitation programs, and community habilitation services. CP Unlimited focuses on giving people the personal, social, and vocational supports needed to live in their communities. CP Unlimited also provides rehabilitation, nursing, vocational services, behavioral, dietary and Article 16 services including counseling, physical therapy, occupational therapy, and speech therapy.

Currently CP Unlimited is seeking Program Coordinators in our Staten Island Community Living Services Programs to develop, implement and monitor the program plans designed specifically for each person that resides in our programs.
Specific Duties for the Program Coordinator include:
Provides training to all direct care staff regarding program implementation.
Supervises staff in the area of programming and data collection. Provides input for staff performance evaluations.
Chairs and participates in all annual, semi-annual and special team conferences for all people receiving services.
Completes all required documentation (i.e., minutes, program plans, and annual updates for each person CP supports). Ensures monthly notes are completed correctly in compliance with agency standards and that the records are maintained and documentation is uploaded accurately for all the people we support.
Ensures integration of services with CP Day Programs.
Attends all staff meetings as scheduled.
Ensures that community inclusion and in-home activities are based on the interests of the people receiving services and are provided on an ongoing basis.
Arranges for necessary supports (i.e. transportation, staff, etc.) Ensures all activities are appropriately documented on an activity log.
Ensures that all required records are maintained on site and are updated as needed.
To qualify for the Program Coordinator :
Bachelor’s degree in related human services field,
Minimum of 1 year experience working with people with intellectual or developmental disabilities,
Supervisory experience preferred.
QIDP status, knowledge of HCBS Waiver and IRA/ICF regulations,
Must be able to work flexible hours as needed,
Working knowledge of Microsoft Office Applications.
