Skills: - Acute care experience in a hospital or clinical setting - Strong patient care skills and ability to provide personalized pharmaceutical care - Knowledge of medical management principles and practices - Proficiency in patient assessment techniques - Completion of a pharmacy residency program preferred - Expertise in pain management strategies and protocols - Experience with IV infusion therapy and medication administration - In-depth knowledge of physiology and pharmacology principles - Familiarity with utilization management processes and strategies Responsibility: - Provide pharmaceutical care to patients in various healthcare settings - Dispense prescription medications accurately and efficiently - Collaborate with healthcare professionals to develop and implement patient care plans - Conduct patient assessments to ensure safe and effective medication use - Monitor and manage medication therapy to optimize patient outcomes - Educate patients on proper medication use, potential side effects, and drug interactions - Participate in medication reconciliation processes - Provide drug information to healthcare professionals and patients - Ensure compliance with all relevant laws, regulations, and standards.
