Responsibilities Job Summary TheHome Care Licensed Practical/Vocational Nurse is responsible forproviding direct nursing care as prescribed by the physician to acutelyill, and/or chronically ill patients in their homes as well as recordingclinical and progress notes; and coordinating other patient careactivities/services to provide the highest quality of patient care inthe geographic service area to which she/he is assigned. She/he worksunder the direct supervision of the Registered Nurse and/or designatedsupervisor. Nursing activities are limited to these in compliance withthe State LPN/LVN Practice Act, any applicable licensure/certificationrequirements and agency policies and procedures. Specific JobDuties/Responsibilities Provides high quality clinical services withinthe scope of practice, as defined by the state laws governing thepractice of nursing, in accordance with the plan of care, and incoordination with other members of the health care team, following allapplicable infection control procedures. Documents patient visitsthoroughly and completely per regulatory and payer requirements in theelectronic medical record utilizing the agency\'s electronic visitverification program (as applicable). Actively participates in theagency\'s care coordination process including timely reportingpatients\' needs and changes in condition, attending patient caseconference, communicating effectively to the patient, family, physician,other clinicians, agency management staff, and case coordinator,community case managers (as applicable) Ensures confidentiality ofpatient health information per agency policy. Education and ExperienceLicense Requirements Must have current LPN licensure in state ofpractice. Current driver\'s license, vehicle insurance and access to adependable vehicle or public transportation. Current CPR certificationrequired.
