　　· Outpatient only

　　· Electronic Medical Record system (Epic)

　　· Ancillary support services available (behavioral health, pharmacy, dietician, care navigation)

　　Licensure & Certifications

　　· Board certified/board eligible in Family Medicine

　　· Licensed in Indiana or eligible for medical licensure in Indiana
