Overview

　　Fairview Health Services is seeking a Family Medicine physician to join our primary care practice at M Health Fairview Clinic – North Branch. Join our busy and growing practice located in the hub of North Branch.

　　Practice Details:

　　13 provider primary care practice.

　　Onsite services including Pharmacy, Physical Therapy, Eye Clinic, and more.

　　Patient encounters average 18-22 patients per day.

　　Openings: 0.8 FTE to 1.0 FTE

　　Full Time is considered 36 patient contact hours per week.

　　Outpatient only position.

　　Call Frequency: approximately 1:20. (low intensity - phone consultation only with first tier nurse triage).

　　Fully integrated Electronic Medical Record (EPIC).

　　H1B Visa accepted

　　Qualifications

　　Graduate of an accredited ACGME/AOA Family Medicine residency training program.

　　BC/BE with the ABMS or AOA.

　　Licensed or ability to obtain licensure through the Minnesota Board of Medical Practice.

　　Ability to obtain and /or maintain DEA certification for the State of Minnesota.

　　Meet threshold criteria/qualifications for Credentialing and Privileges (if required).
