　　Job Summary and Qualifications

　　The Physical Therapistcarriesout individualizedtherapyprogramsincluding assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The PTsupervises therapy assistants,aidesand students.We are an amazing teamthat works hard to support each other and are seeking a phenomenal addition like you, who feels patient care is as meaningful as we do.

　　What you will do in this role:

　　You will perform comprehensive patient assessments andestablishappropriate treatmentgoals and programs for patients in Acute care, Inpatient Rehab, and/or Outpatient.

　　You will coordinate the treatment program with other clinical/therapeutic services.

　　You will develop comprehensive goals for treatment to maximize patient outcomes.

　　You will deliver care andmodifythe care planin accordance withthe patient’s needs.

　　You will serve as an educator to patients, families,studentsand colleagues.

　　What Qualification You Will Need:

　　Minimumeducationrequiredis aBachelor’s Degree in Physical Therapy/Master’sdegree preferred.

　　Current State professional license or license eligible.

　　Good interpersonal, organizational and leadership skills and the ability to communicate effectively both verbally and in writingrequired.

　　Basic Life Saving (BLS) – American Heart Association or American Red Cross

　　Newgradsaccepted.

　　Excellent verbal, written, and presentation skills; Diagnostic skills consistent with professional degree

　　Proficient with computer software programs.

　　Proficient with patient care related equipment
