Team 7 covers Northern Suffolk; Centereach, Head of the Harbor, Lake Grove, Nesconset, Nissequogue, Old Field, Port Jefferson, Port Jefferson Station, Setauket, South Setauket, St. James, Stony Brook, and Terryville and the surrounding areas. 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends Great Healthcare benefits, and education reimbursement of $7,000 annually Additional compensation for visits over your caseload Job Details Our Home Care PTs: Review patient charts and medical history Develop a personalized treatment plan Teach therapeutic exercises and stretches to improve patient’s conditions Use hands-on therapy to promote healing Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload. Provide patient and family education for health promotion as directed by physician’s plan of treatment. Provide care to the most upmost standard Minimum Qualifications: New York State PT license Minimum of one year of recent hands on experience as a Physical Therapist. Valid New York State Driver’s License and reliable transportation Salary Range USD $90,000.00 - USD $110,000.00 /Yr. The pay for this position ranges from $90,000 – 110,000 Annually. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
