　　Patient Care Coordinator- pharm tech (intake) Key Responsibilities: - review contents of referral to ensure complete and accurate info received - create patient profile from a scan or fax image by entering correct patient and provider info - conduct initial check of prescription to ensure clinical accuracy for processing - communicate with provider regarding referral status, prescription clarification, missing info, or patient concerns - updating concise progress notes to ensure other roles within the workflow can quickly assess the status of referral - enter in new provider information Qualifications / Requirements: High school graduate or equivalent 2 years of experience in Pharmacy Pharmacy Technician License
