POSITION TECHNICALRESPONSIBILITIES Order Entry 1. Enter medication orders into (or performfinal quality control check if performed by technical personnel)Meditech computer system and initial each order sheets to indicatepharmacist has reviewed the order(s). 2. Review all new orders forappropriateness (date, route, frequency, indication), duplication oftherapy, safety (e.g. drug-drug interactions, drug allergy) 3. Resolvedrug therapy problems identified by intervening with the appropriatephysician or health-team member; change orders when appropriate. 4.Clarify all incomplete, inaccurate, or unclear orders with appropriatephysician or health-team member. 5. Perform all necessary dosagecalculations. 6. Calculate and pursue dosage adjustments for patientswith impaired renal or hepatic function. 7. Perform and documenttherapeutic interchanges or formulary substitution on appropriateagents. 8. Document clinical interventions in computer system. PatientCare 1. Review all patients\' computerized medication profiles-interveneif a drug therapy problem is identified. 2. Provide drug information forrequests immediately affecting patient care. 3. Perform drug monitoringconsults (i.e. pharmacokinetic consultations). 4. Manage TPNorders-review and document labs and perform necessary interventions. 5.Perform therapeutic drug monitoring on appropriate drugs (review andassess appropriate labs and drug levels; e.g. warfarin, anticonvulsants,digoxin). 6. Review all patients\' labs and microbiology cultures andintervene with appropriate physician if necessary. 7. Patient relatedservice documentation (e.g. performing and documenting 1st doseteaching, diabetes teaching, warfarin reaching, discharge counseling).Other 1. Guide and perform final quality control check of duties thatmay be performed by technical personnel: drug selection, preparation,packaging, labeling, etc. including unit dose cart check. 2. Prepare orsupervise/check the preparation of parental sterile products; includinginjections, IV admixtures, TPN. 3. Instruct on the proper methods ofadministering drugs and precautions/side effects involved in their use.4. Insure timely delivery of medications to all patient populationsserved. 5. Assist with the training of new department employees. 6.Provide drug information and pharmacotherapy recommendations to otherhealthcare professionals. 7. Maintain proper drug storage, recordkeeping, and utilization data as required by state and federalregulations and good practice standards. POSITION REQUIREMENTSEducation: Required: PharmD degree or Bachelor\'s degree in Pharmacywith equivalent experience. Preferred: None Experience: Required: 1-2years of hospital experience as a Pharmacist Preferred: None Licensesand Registrations: Required: State of Wisconsin license as a PharmacistPreferred: None Certification(s): Required: Basic Life Support (BLS)certification within 3 months of hire and Advanced Cardiac Life Support(ACLS) within 6 months of hire.
