  ```- Team Members working within the community are paid mileage for the use of their vehicle. Participants requiring transport will be transported by company drivers and company vehicles.

  Consistent schedule, hours and income, and overtime potential.

   

  Job Summary:

  Provide personal care and assistance to Participants in the center, home, or domicile care setting. Work with other Team Members as required in implementing and carrying out services and activities and meeting the needs of individual Participants.

   

  Duties/Responsibilities:

  Provides supervision, personal care, and basic housekeeping to Participants in the center, home, or in the community, which may include but is not limited to:```{=html}

  ```- Bathing, dressing, feeding, toileting, transfers, lifting, and incontinence maintenance

  Laundry, meal preparation, clean-up, and other general cleaning```{=html}

  ```- Travels to and from Participant's homes using the aide's own properly registered, inspected, and insured car.

  Accompany Participants to appointments as needed.

  Medication monitoring and prompting as appropriate.

  Responsible for reporting any changes in Participant's medical or physical status to appropriate Team Leader or other appropriate medical Team Members.

  Documents services provided and observations on appropriate forms using Alaya. May require personal cell phone use (will be reimbursed with proper documentation and expense filing).

  ```- Works one-on-one with assigned Participants to implement specific activity plans.

  Provides care to Participants per Interdisciplinary Team plan of care.

  Prepares Participants for treatments.

  Participate in and support Quality Improvement initiatives.

  Other duties as assigned by Center Manager or designee.

   

  Supervisory Responsibilities:

  None.Required Skills/Abilities:

  Relates well with people of all ages but has a deep sensitivity and commitment to the elderly

  Ability to organize work and follow through on assigned tasks

  Strong verbal communication skills and can follow written and/or follow oral direction

  Must possess the ability to make independent decisions when circumstances warrant such action

  ```- Must be able to deal tactfully and patiently with personnel, Participants, family members, visitors, government agencies/personnel, and the public.

  Must have the cognitive ability to process and perform tasks for Participant safety.

  CPR and First Aid Certification.

  Must be a minimum of 18 years of age

   

  Education and Experience:

  High School Diploma or GED

  One year of experience working with the frail and elderly population or willing to complete Company paid training to meet this requirement

  Must possess a valid PA driver's license, vehicle, and clean driving record

  Good interpersonal kills and the ability to work with geriatric patients and handle unusual interpersonal situations

   

  Physical Requirements:

  Must be able to move intermittently throughout the workday

  Must be able to lift, push, pull, and move a minimum of 25 pounds

  Must be able to assist with the evacuation of Participants

  ```- This position may encounter issues/infestations in the homes of the Participants. Following proper protocols is essential.

  Employment Requirements: Successful completion of Pennsylvania Criminal History Check, PA DHS Child Abuse Clearance, Office of Inspector General check, and FBI Fingerprinting for Child Protective Services Law (at employer's expense). Successful completion of post-offer employment drug screening and physical (at employer's expense). CPR and First Aid Certification, or willing to obtain at employer's expense. Individuals must have one year of prior experience caring for the frail elderly or willing to complete Company paid training to meet this requirement.

  This position has full access to Participant, program, and staff records and reports, requiring discretion to protect individuals and the program.
