OVERVIEWUnder the CPA Resettlement Program, recovery support services will be provided to citizens returning to the Greater Hartford area. The goal is to provide reentry services that assist individuals address their substance abuse and mental issues in order make a successful transition back to their community. CPA is collaborating with InterCommunity Inc. to provide substance abuse and/or co-occurring treatment and recovery services. Participants served will be women and men 18 years of age and older who: have been assessed/diagnosed as having a Substance Use Disorder (SUD) and/or co-occurring substance and mental health disorder; have been sentenced to and serving at least three months in prison or have violated parole and serving at least one month in prison; be within four months of release to the community; and upon immediate release from prison be connect to treatment.
POSITION REQUIREMENTSReporting to the Program Manager, the Peer to Peer Case Manager will provide Pre and Post-Release wraparound reentry support services. Bachelors degree plus two years of experience in social services field. Equivalent work experience will be considered in lieu of education. Knowledge of criminal justice, court systems and community providers that offer services to the criminal justice population. Demonstrate the ability to interact with people of diverse backgrounds. Must have strong crisis management skills as well as strong written and oral communication skills. Experience conducting assessments, performing case management functions and meeting the needs of multi-cultural population preferred. Strong computer skills. The ideal candidate will be in recovery for at least five years and/or formerly incarcerated. If formerly incarcerated the individual must have completed parole and/or probation and have no criminal justice involvement for at least five years. Must have a valid drivers license. One position available; bi-lingual (English/Spanish) preferred.
KEY RESPONSIBILITIESI. Pre-Release Case Management - minimum 2 months per participant:
Conduct weekly visits to Cybulski Community Reintegration Center (Enfield, CT) and York Correctional Institution (Niantic, CT) to recruit/enroll eligible people for the program;Facilitate participant orientations and administers assessments utilizing Motivational Interviewing techniques;Develops individualized reentry/treatment plan based on assessments;Assists participants with securing entitl ments and ensures they are active upon release;Coordinates participants transition back to the community and connects them to InterCommunity for treatment and other necessary community services (e.g. housing) immediately upon release.II.
