Job Summary:
The Patient Navigator for the NBI ED-ALT Grant is responsible for effectively guiding patients who enter the Emergency Department with chronic and/or acute pain access resources needed to manage their pain and meet their social needs. The Patient Navigator will conduct pain assessments, screen the individual for social determinants of health, determine the most appropriate services to address their needs and educate the patient on their options. The Patient Navigator will assist in removing any barriers associated with access to care. The Patient Navigator will manage follow-up appointments to coordinate continuation of care and linkages to community resources. The Patient Navigator will also work alongside the Clinical Leads to support the patient and ensure the highest quality of care.
Job Responsibilities:
Conduct pain assessments to gauge patients' pain experience and develop plan with medical staff that serves the patients' needs while in the hospital system and post-discharge
Conduct Brief Pain Inventory with patient
Conduct Pain Catastrophizing Scale with patient
Screen for social determinants of health and provide patient navigation services to community resources
Screen for substance-use disorders and provide warm hand-off to substance-use treatment services
Collaborate with system and community-based partners to ensure coordination of care
Educate patient on alternative therapies to pain management
Educate patient on MOUD and multiple pathways to recovery from substance-use disorder
Participate in multidisciplinary team meetings
Present to medical team on opioid sparing order sets, adjunctive therapies for pain diagnoses
Perform other duties as assigned
Education/Experience:
Bachelor's degree in behavioral health profession required; Master s degree preferred
LCADC, LPC, LCSW, LMFT, or Licensed Psychologist preferred
Strong communication skills, active listening skills, cooperation and teamwork
Must possess the knowledge, skills and experience necessary to completely perform case management
Minimum of two years demonstrated experience working in an emergency room setting
Demonstrate a commitment to diversity, equity, and inclusion through continuous development/training, modeling inclusive behaviors, and proactively managing bias
Schedule:
Monday - Friday 8:00am - 4:00pm, subject to change based on department needs.
