Major Responsibilities/Activities:
Provides casework and case management services to families;
Conducts evidence-based assessments;
Assess and monitor safety, imminent risk, and well-being of children;
Assess family needs in order to establish stable functioning and a safe environment;
With the parents, develop a plan to encourage and strengthen a meaningful relationship between the parents and children;
Retain up-to-date case records and progress notes on Connections in accordance with NYS regulations;
Engage in regular contact with the referral source and coordinate with other agencies and community service providers;
Plan and arrange for case conferences and service plan reviews;
Function as a member of the PS team, attend weekly meetings, offer expertise and resources, and actively participate in case conferences;
Maintain casework contacts as mandated by county regulations, a minimum of two face-to-face contacts with family members especially all the children;
Models and practices sensitivity, fair treatment, and acceptance of diversity in all interpersonal interactions;
Maintain positive working relationships with program staff and others within the agency. Demonstrate strong interpersonal skills during interactions with internal and external customers.
Minimum Requirements:
Bachelor’s Degree in Social Work, Psychology, Human Services or related field
Experience working with children and families
Experience in case management is a plus
Bilingual speaking is a plus
Valid NYS Driver’s License, reliable vehicle, and adequate insurance
Competitive Pay Range of $40,000-$42,000 annually based on experience
