　　This Patient Navigator Will Be Responsible For

　　Supporting a mental health team in a specialty care medical setting

　　Helping with care coordination, referrals, and connecting patients to financial, housing, legal, mental health, transportation and food resources

　　Helping families navigate the healthcare system

　　Helping families complete and submit summer camperships

　　Assisting the mental health team with program development projects aimed at improving patient care

　　Participating in program meetings

　　Assisting the mental health team with scheduling needs

　　Coordinating program support groups

　　Documenting work in the medical record

　　In Order To Qualify, You Must Have

　　High School Diploma / GED required, Bachelor’s Degree in Human Services or other related field preferred

　　Interest in working with patients and families with complex needs, in particular social determinants of health

　　Fluency in Spanish is preferred

　　Strong communication and interpersonal relationship skills with the ability to respond to events effectively.

　　Access to a private location to work from home or offsite (this is a hybrid position with regular on-site time required).

　　The ability to work in a fast-paced environment, pay high attention to detail, to communicate effectively both orally and in writing, and strong organizational skills

　　Expertise with building community partnerships, trauma-informed care, mental health services, social determinants of health, housing policy and resources, and food insecurity

　　The ability to be flexible and have the willingness to learn
