　　Service Provision Evaluation Services and Therapy

　　Complete client intake/assessment to include history, presentingproblems and diagnoses using DSM-V criteria

　　Identify need for Individual therapy, family therapy, CBS services, orSED waiver services using identified and accepted methods to remediatesymptoms of mental illness and/or substance use treatment to improveemotional and functional well-being as needed

　　Conduct risk assessments as needed to address safety of clients

　　Demonstrate flexibility in adapting to workflow when completingscheduled and unscheduled work assignments

　　Initiates outreach to increase engagement

　　Provide Group and/or Individual therapy as needed using identified andaccepted methods to remediate symptoms of mental illness and/or improveemotional and functional well-being

　　Treatment Planning

　　Prepare quality treatment plans in collaboration with client and/orguardian

　　Facilitate treatment plan review, addressing updates and progress ongoals with clients treatment team if applicable

　　Documentation

　　Complete progress notes, intake summaries, risk assessments and agencyreferral documentation within 48 hours of meeting with a client tocomplete an intake for services

　　Complete progress notes in accordance with CCBHC and COMCARE guidelineswhich meet requirements for medical necessity, goal-directed treatmentand client response to treatment within prescribed timelines.

　　Complete Notes for Record, precautions, authorization for release ofrecords, releases of information, safety documents and otherdocumentation as required and/or recommended within establisheddeadlines

　　Enters intake cancellations for accuracy and completeness of medicalrecord
