  Description

  LOCATION: Herberger Center – Phoenix, Arizona

  STATUS: FTE/40 hours/wk

  MISSION STATEMENT:

  The Salvation Army Phoenix Family Services is looking to fill an Outreach Specialist for Project Hope. The successful candidate for this position will have the ability to work independently but also coordinate services with other team members, demonstrate appropriate crisis intervention skills and have an awareness and connection to community services.

  The Outreach Specialist will provide information and outreach services to homeless individuals and families from outreach areas and where the homeless congregate. This will include connection to shelter, transportation needs and other basic services. This position will complete all required case management documentation, including travel logs, client services and information pertaining to grant and TSA statistical requirements. This person will maintain and build on current relationships with partnering agencies.

  QUALIFICATIONS:

  Associate of Arts Degree in Human Service or equivalent combination of education, training, and experience

  Ability to work independently with limited supervision, but also coordinate services with other team members

  Ability to relate to people in crisis, who suffer from Serious Mental illness, chemical dependency, physical disability, chronic homelessness, or diverse cultures

  Demonstrate appropriate crisis intervention skills

  Possess time management skills, appropriate judgment in crisis situations, detail oriented and organized

  Awareness of and connection to community resources

  Maintain the national social service standards of TSA by applying these policy and procedures and best established practices

  Pass a criminal background check and have a good driving record

  Must possess a current AZ Driver’s License

  PHYSICAL REQUIREMENTS:

  Qualified individuals must be able to perform the essential duties of the position with or without accommodation. The Salvation Army will attempt to satisfy requests as long as the accommodation needed is reasonable and no undue hardship would result.

  While performing the duties of this job, the employee is regularly required to walk, stand, climb or balance, stoop, kneel, crouch, crawl and reach with hands and arms on a continuous basis. The employee must regularly lift and/or move up to 20 pounds, frequently lift and/or move up to 50 pounds, and occasionally lift and/or move up to 75 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, depth perception and ability to adjust focus. Must have the ability to operate telephone, a desktop or laptop computer, as well as the ability to access and produce information from a computer, and to understand written information.
