Job Summary
Under general supervision, the MEND Nutrition Leader (through the MEND -"Mind, Exercise, Nutrition - Do It!" Program) will empower andsupport children and their parents/caregivers to make healthy lifestylechanges and choices that help them manage chronic medical conditions,improve the quality of their life, and reduce their health risks. The MENDNutrition Leader is responsible for coordinating and leading the nutritioneducation portion of the MEND and Tasting with Tots program and providingindividual and family coaching and advising in the Healthy Lifestyles Clinic.
Essential Functions:
Program ImplementationManages planning and delivery of the nutrition education portion of the MENDand/or Tasting with Tots programs, including choosing and teaching weeklycurriculum topics, and adjusting curriculum as needed to fit needs ofparticipating groups and individuals. Selects healthy snack and other suppliesto accompany each lesson and prepares snacks and equipment for classes.Prepares the venue and cleans up after each session. Assists with fooddistribution with Denver Food Rescue partnership.(30%)Patient recruitment and retentionManges and fills program schedules by calling patients and managing referrals.Adjusts templates as needed based on current no show rates to ensure adequateprogram participation. Updates both clinic and program staff on currentprogram wait times and availability.(20%)Nutrition AdvisingProvides tailored nutrition advise and health coaching to families andindividuals in all programs. Answers patients questions regarding appropriatehealthy lifestyle changes.(20%)Strategy and PlanningAssist in the implementation of short- and long-term priorities of the team.Partners with leadership to implement strategic plans to achieve maximumdepartment output and contribution to Denver Health strategy. Identifiespotential community partnerships based on patient needs.(15%)Quality ImprovementCollaborates with other program staff and managers to identify areas ofimprovement and growth based on experience with programs and patients.Utilizes established processes (e.g., Lean) philosophy and methodologyto continually improve standard work and processes.(15%)Education:Associate's Degree RequiredWork Experience:
1-3 years Considerable experience working with children and families, andleading groups. Required and1-3 years Experience in dietetics, nutrition, children's health,behavioral change and/or physical activity ideal. RequiredLicenses:
Knowledge, Skills and Abilities:
Ability to motivate and inspire meaningful change in groups of children andparents.Ability to comply with relevant child protection guidelines.Able to take all reasonable steps to ensure the safety of children attendingthe program.Ability to act in accordance with the law and all applicable regulatorycontrols.Ability to conduct themselves and staff to act at all times in a professionaland diligent manner that befits working with and holding a position ofresponsibility over children.Ability to work well as a team, and show respect for patients and familiesas well as staff.
