　　Job Description

　　LG Health Physicians Family Medicine is seeking a part-time Nurse Practitioner or Physician Assistant to join the well-established Metro region, a collection of 5 primary care practices geographically oriented close to Lancaster City. This position is for a regional “float” provider, which would primarily offer same day acute access at multiple locations on a recurring basis to complement our conventional practice clinicians. This role is not responsible for actively managing an established patient panel. The position and supervising physician are based at FM Manor Ridge.

　　Our family medicine practices provide comprehensive care for patients of all ages. We emphasize wellness and prevention, help patients manage chronic conditions, and provide timely care in times of illness or injury. We are an accredited Patient-Centered Medical Home that offers additional support to help patients establish goals and manage their health, connect to community resources, access to on-site addiction medicine treatment and counseling services

　　Metro Region Primary Care Practices include: East Petersburg, Internal Medicine, Crooked Oak, Manor Ridge & Abbeyville.

　　Hours: Part-time hours available (20-32 hours/week)

　　Job Requirements

　　Nurse Practitioner

　　Valid Certified Registered Nurse Practitioner license and Certificate of Prescriptive Authority in Pennsylvania

　　Valid Pennsylvania licensure as a Registered Nurse

　　Certification by ANCC or other qualifying agency in area of specialty required

　　Family Medicine experience preferred

　　Physician Assistant

　　Current licensure as a Physician Assistant by PA Board of Medicine

　　Graduate of Accredited Physician Assistant training program

　　Family Medicine experience preferred
