JOB REQUIREMENTS: Position Description: Position Purpose AdvancedPractice NurseProvides primary or specialty health care services withindefined scope of practice in collaboration with a licensed physician andin accordance with MCW/FMLH/CHW policies and procedures. The APP/NP workin partnership with all Pediatric Hospital Medicine physicians, theMedical Director of the PHM-FV program and the Assistant SPU Chief, whoserve as the primary collaborating/supervising physicians, the MedicalDirector of PHM-FV, Assistant SPU Chief-FV, Director of PHM-MKE and theDivision Administrator II - PHM-MKE. Core Responsibilities Performscomprehensive histories and physical exams, identifies normal andabnormal findings, including physical, cognitive and psychosocialstatus. Orders and interprets laboratory and diagnostic tests. Developspharmaceutical and non-pharmaceutical treatment plans. Monitorseffectiveness of plan of care. Educates patients on disease andtreatment plans. Performs procedures as trained and appropriatelycredentialed. Documents within the Electronic Medical Record (EMR) andcompletes other necessary documents. Collaborates and/or consults withphysicians and other health care providers as appropriate. Educates,counsels, and assists patients and families regarding current healthproblems, health maintenance activities and preventive behaviors.Department Specific Responsibilities Patient Care: Collaborate withCW-FV PHM team and interdisciplinary health care teams in caring fornewborns born and cared for at Thedacare-Neenah hospital as well as:Children presenting for infusions/short stay or other care at CW-FVhospital Order and interpret diagnositic tests, procedures, laboratorystudies and professional consultations. Appropriately communicatefindings with the medical team as well as with patients and families.Develop and implements treatment plans in collaboration with the PHMattending, monitor effectiveness of plan of care throughout the patientshospital stay, coordinate the care and discharge of patients. Documentwithin the EMR accurately and timely the Admission, Progress anddischarge notes as well as orders and discharge instruction as well ascommunicate with the PCP as needed. Support the infusion clinic withtreatment plan implementation, order entry and participate in meetingsregarding this clinic.
