　　Nurse Practitioner - Medical Office

　　Location: Carelon Health Medical Office - 3041 E. Flamingo Rd., Las Vegas, NV.

　　*May see patients at the three different Carelon Health Medical Offices in Las Vegas and Henderson, NV.

　　Work Shift: 5 Days a Week, Monday – Friday: 8:00 am - 5:00 pm Or 4/10 Days, 4 days, 10 hrs. work shift per day.

　　The Nurse Practitioner is responsible for managing patient care and treatment (in person, telephonically and/or video) in a primary care setting.

　　How will you make an impact:

　　Perform health assessments, preventative health measures, chronic and complex care management within prescribed guidelines and physician collaboration.

　　Order, interpret and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs.

　　Collaborate with physician partners.

　　Formulate and document care plan.

　　Prescribe medication or other forms of treatment.

　　Completion of quality measures.

　　Participate in a rotating On-Call program, which may include after-hours, weekend and holiday calls and visitations.

　　Minimum Requirements:

　　Requires an MS in Nursing and minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.

　　Current, unrestricted RN license and NP license in the state of Nevada is required .

　　Possession of DEA registration or eligibility.

　　For Carelon Health business units, satisfactory completion of a Tuberculosis test is a requirement for this position.

　　Preferred Skills, Capabilities and Experiences:

　　Experience in a Primary Care setting with adult and geriatric patient population.

　　Advance Care planning experience.

　　Experience in Wound Care.

　　Bilingual communication skills in English and Spanish.

　　For candidates working in person or remotely in the below locations, the salary* range for this specific position is $108,108 to $162,162.
