　　Overview

　　The Nurse Practitioner will be responsible for caring for patients, maintaining accurate and current patient records and scheduling and administering follow-up appointments to patients as required. Work as a member of the Nursing Team to coordinate services with our nurses and physicians and will assist in delivering premium care to each and every patient.

　　Responsibilities

　　Essential Duties and Responsibilities

　　Perform initial patient screenings and patient history and physical examinations

　　Provide infusions and injections to Mosaic patients

　　Provide complete assessments of patients receiving infusions, and initiate emergency protocols should significant adverse reactions occur

　　Deliver quality care while maintaining productivity metrics

　　Update patient records and check records for accuracy at each patient appointment

　　Analyze test data and determine the need for follow-up appointments and further treatment options

　　Serve as the resource patients need for ongoing care information, and counseling, and provide guidance for any patients with critical conditions

　　Work collaboratively with physician to deliver optimum care to patients

　　Work within a team and take direction

　　Complete documentation in Company EHR system

　　Utilize strong written and interpersonal communication skills

　　Qualifications

　　Master’s Degree in Nursing

　　5+ years’ nursing experience

　　Valid nurse practitioner license
