JOB REQUIREMENTS: Duties The CRH is a VISN-owned resource that providesprimary care, mental health, and specialty care access services tofacilities within VISN 7. The CRH nurses use the nursing process andevidence-based practice to collaborate with interdisciplinary teams todevelop and implement patient-driven care plans. The CRH nurses are partof the Mobile Deployment Team, providing care both in-person and virtualcare. The incumbents are under the supervision of a NurseManager/Associate Chief Nurse or other CRH Supervisor who provides theadministrative supervision and general instruction, plans and assignswork, assigns time schedules, approves leave, and completes performanceevaluations. Clinical supervision may be provided by a Registered Nurse,Physician Licensed Independent Provider (LIP.) Supervisoryaccountability for nursing practice is to the VISN 7 CRH Associate ChiefNurse. Enhancing patient safety and quality of care by collaboratingwith primary care/specialty care clinicians to develop, oversee, andmanage care management plans and care coordination for patients assignedto one or more primary care/specialty care teams. Participating in modesof communication and care delivery including, but not limited to, dailyhuddles to discuss patient care, secure messaging, telephone care, viewalerts management, shared medical appointments, VA Video Connect (VVC),Clinical Video Telehealth visits (CVT), face to face visits, etc. Aspart of care management, identifying patient needs for the involvementof discipline-specific team members and discussing nursingrecommendations with the Primary Care Physicians (PCP) and otherclinical team members. Engaging relevant primary care staff to supportnursing care, according to locally established informal and formalcommunication processes, including entering consultation requests todiscipline-specific primary care/specialty care members, if required forformal communications. Partnering with disciplines such as ClinicalPharmacy Specialists, Primary Care Mental Health Integration teams, andother specialty teams, to provide team-based, group, and individual careto patients with chronic diseases. Assuming full responsibility forassignments to Clinical Associates or Administrative Associates relatedto care management, care coordination, nursing services, and outcomes ofcare.
