　　This role is a professional RN with service line specific clinical knowledge who offers individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. Assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on high-risk patients or those with complex care needs (including multidisciplinary care). This role has a particular focus on improving transitions in care and adherence to care and functions as a clinical advocate and educator for patients.

　　MINIMUM QUALIFICATIONS:EDUCATION, CERTIFICATION, AND/OR LICENSURE:1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurseORCurrent multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).2. Obtain certification in Basic Life Support within 30 days of hire date.EXPERIENCE:1. Three (3) years of nursing experience.

Assesses and documents patient's needs, goals, and preferences and work to ensure such are integrated into treatment and care delivery.8.
