We are seeking a Nurse Care manager to work in collaboration with team members to serve children and families in our Children's Behavioral Health Homes program. The Nurse Care Manager works in conjunction with the multidisciplinary children's BHH teams to coordinate and provide interventions to improve quality of care and health outcomes for clients, to support integrated care efforts, and ensure program compliance with required health metrics. The ideal candidate will be highly organized and collaborative and will serve clients across various locations.
This position is eligible for a hybrid work model and the pay range is $66,500 to $79,000 depending on experience.

DUTIES:
Participate as a member of a multidisciplinary team by attending client meetings, consultations, internal team meetings, and external stakeholder meetings.
Provide education to clients and family/caregivers, as well as team members, about health and wellness, safety factors, and medications, among other topics.
Communicate with clients' PCPs upon admission and as needed to gather information pertaining to annual physicals, medications, health conditions, and to discuss any significant healthcare needs or gaps in care.
Review and monitor HIN and VMS for pertinent participant health information.
QUALIFICATIONS:
Must be a registered nurse; a psychiatric nurse licensed as a registered professional nurse by the state where services are provided and certified by the American Nurses Credentialing Center (ANCC) as a psychiatric and mental health nurse; a PMH-APRN who is licensed as a nurse practitioner or clinical nurse specialist by the state where services are provided, who has graduated from a child and adolescent or adult psychiatric and mental health nurse practitioner or clinical nurse specialist program and is certified by the appropriate national certifying body; or an advanced practice nurse, as defined by the Maine State Board of Nursing.
