  Overview

  St. Luke’s Palliative Care is seeking a full-time Advanced Practitioner to join their highly collegial and patient-focused team. This team provides coverage in the Boise, Meridian, and Nampa, Idaho hospitals and has two clinics in Boise and Meridian. The selected individual will be working with a team of 6 physicians and 5 other APPs, assisting patients and their families through any stage of a serious illness or health problem, discussing treatment choices, decision-making, and managing their illness at home to avoid hospitalization. In this role, our providers work both in the hospital and in the clinic to provide this care and services.

  Shifts can either be set at 4/10s or 5/8s with minimal call coverage.

  *Minimum of 3 years experience is required.

  The Advanced Practice Provider (Physician Assistant or Nurse Practitioner) is responsible for providing high-quality healthcare through the diagnosis and treatment of patients with conditions ranging from mild to acute and/or chronic. The APP works under the supervision of a designated physician and in collaboration with a team of other providers and staff.

  JOB REQUIREMENTS INCLUDE:

  Graduation from an accredited school with a minimum of a Master’s Degree and certification, as required by the specialty, as well as eligibility to obtain licensure from Idaho to practice as a Nurse Practitioner

  Must obtain and maintain unrestricted prescriptive authority, DEA, and controlled substance licenses.

  Excellent communication skills to include oral and written comprehension/expression.

  Ability to manage a fast-paced, busy clinical environment related to changing patient needs, including work with patients with acute, chronic, and complex disease processes.

  Ability and willingness to exhibit behaviors consistent with standards for performance improvement and organizational values (e.g., efficiency & financial responsibility, safety, partnership & service, teamwork, compassion, integrity, and trust & respect).

  Responsibilities

  The Nurse Practitioner (NP) is responsible for promotion of health, prevention of illness, and the response of the human condition through diagnosis and treatment. The NP works under the supervision/sponsorship of designated physician(s) and in collaboration with a team of other providers and staff; promoting a culture of collaboration and support between all providers within the St. Luke's Clinic Medical Group and to support the model of team based, patient-centered care endorsed by physicians and advanced practicing providers within our medical group.

  -Provide care through diagnosis, clinical judgment, anticipatory guidance, and management to restore, maintain, and improve the health status of the patient. -Perform history and physical exams, acute and episodic care, orders pertinent lab and diagnostic studies with interpretation and consultation. -Initiate and manage appropriate pharmacologic, non-pharmacologic agents, and treatments within the scope of practice. -Establish and document the healthcare plan and treatment, maintaining records of each case that are sufficiently complete for any healthcare professional to evaluate and follow the treatment plan and subsequent outcome(s). -Recommends strategies to assist SLHS in meeting its critical goals and objectives in the most efficient manner possible; administers recurring or daily functions or processes necessary for systems execution and identifies opportunities for improvement or efficiencies.'-Interact with patients/families in a professional and sensitive manner, and maintain confidentiality. -Maintain professional relationships and convey relevant information to other members of the health care team within facility and any applicable referral agencies. -Accept assignments based on patient census, acuity, needs, and the qualifications and competencies of self and of other staff members. -Attend staff meetings, in-services, and continuing education. Report to work on time and as scheduled. -Contribute to annual reviews of peer performance as requested, assist in data collection, study methods and other development as assigned. -Perform other duties and responsibilities as assigned, or as pertains to the position and specific department/specialty.

  Qualifications

  Education: Graduate education from an accredited Nurse Practitioner program or qualifying education as designated by the Idaho Board of Nursing.

  Experience: 0 years relevant experience.

  Licenses/Certifications: License and certification, as required by the specialty, or be eligible to obtain licensure from Idaho Board of Nursing to practice as a Nurse Practitioner. Must obtain and maintain unrestricted prescriptive authority, DEA, and controlled substance licenses
