DESCRIPTION:
Delivers direct services such as crisis intervention, case management, counseling, adjunctive therapies, and screening. When necessary and applicable to program guidelines, serves as a mental health consultant and performs off-site activities. Services are delivered in consideration of age appropriate/developmental needs.
ADDITIONAL RESPONSIBILITIES:
Conducts history and initial data gathering from patient, family members, significant others, and other agency personnel
Manages client/family contact via telephone including: initial contact, hotline, screening, and follow up
In consultation with the treatment team, contributes to the psychosocial assessments
In consultation with the treatment team, contributes to the initial treatment plans
In consultation with the treatment team, contributes to the comprehensive interdisciplinary treatment plans
Provides the following services per program s policies and procedures: case management, counseling, adjunctive therapies, psycho educational groups, screening, and parent education
Prepares discharge plans in consultation with Coordinator/Program Director
Implements discharge plan and follow up in consultation with Coordinator/Program Director
Responsible for documentation in the patients MR per program s policies and procedures
Escorts patients to and from appointments, scheduled activities off site and program field trips per treatment plans.
Establishes and maintains supportive relationships with clients
Participates in all crisis intervention, crisis management action per program s policies and procedures
Participates in case conferences, staff meetings, case presentations, Grand Rounds, off site and onsite training as assigned
Follows established departmental/hospital policies and procedures/objectives, quality assurance, safety, environmental and infection control standards
Meeting productivity assignments
Meets customer satisfaction goals
If job assignment is Residential Services/Supportive Housing, assists in preparation of clients weekly medication boxes and supervises clients in self-administration of medication as per departmental policy.
REQUIREMENTS:
Bachelor degree in psychology, Social Work, Counseling or related field,
Candidate must have a minimum of 3 years experience POST bachelor degree working full time in the behavioral health field,
Candidate must have a valid driver license and ability to drive agency vehicles during outreach calls Some job responsibilities may include: assessment of consumers in the community, follow up and linkages to community services after discharge

$21.59 to $26.53 Hourly Rate
