Facility:Dayton Children's - Main CampusDepartment:Social WorkSchedule:Part timeHours:8Job Details:This Medical Social Work position is a pool position providing after-hours and/or weekend coverage for medical social work services at Dayton Children's Hospital. This position serves as an integral part of the after-hours team in the Integrated Care Department, serving patients in the emergency department and inpatient settings. The position works hours as needed to meet department staffing needs for 24/7 coverage, up to 16 hours per pay period.
The Medical Social Worker is responsible for working in a team structure with the RN Case Manager and other Integrated Care department staff, to effect overall care coordination for the transition of care and sophisticated discharge planning process. Through psychosocial assessment and intervention strategies, is responsible for assisting patients and their families in understanding and learning to cope with illness and disability. Assists families in resolving complex personal and environmental difficulties which interfere with effective social functioning and acquiring maximum benefit from medical care.
Participates as a professional team member in patient care planning along with the hospital and community agency personnel. Provides education and consultation to health care professionals and the community regarding the impact of psychosocial factors. Social Workers provide additional support with onboarding, training, and job shadowing for newer staff and students, participate in the department’s peer mentoring program developing social work skills and complete additional special projects as requested.
