Descriptive Title:Medical Case Manager
Full-time, Monday to Friday, 9:00am - 5:00pm, 37.5 hours per week.

Description of Duties:
-Provide comprehensive case management and referral services to HIV infected adolescents and young adults , including some who are of trans experience -Participate in case management, CQI, case presentation, general staff and supervisory meetings -Maintain documentation of case management activities in electronic medical records -Develop and implement assessments and reassessment service plans for assigned clients on caseload -Other duties as assigned by supervisory staff
Qualifications:
REQUIRED: High school diploma, 5 year experience providing case management services to HIV Clients, youth from communities of color and women. Must be able to engage adolescents and young HIV and sexual health prevention education. PREFERRED: Associates or Bachelor's degree Experience working with adolescents and young adults in a variety of capacities Experience providing HIV and sexual health prevention education
