Position Summary
The Medication Assisted Treatment (MAT) Services Coordinator is responsible for the implementation, coordination and facilitation of medication assisted treatment (MAT) program services. This coordination is between clinical providers, remote clinicians or services, clinical support staff such as nurses and case managers and patients. The MAT Services Coordinator is responsible for providing coordination and support to clinical providers and staff on medication-assisted treatment, including developing protocols, enhancing processes, assisting with patient care, and improving clinical outcomes. Other duties include establishing or enhancing agreements between entities and building strong bidirectional referral relationships with key community substance use treatment and other providers. The MAT Services Coordinator is an integral member of the Patient Care team. The Treatment Support Services Coordinator reports directly to the SVP of Medical Services.
Essential Functions, Duties and Responsibilities
Medication-Assisted Treatment/Substance Use Disorder
Assist with the development of protocols, forms, and processes required to ensure quality and safe MAT services to patients at all sites.
Coordinate with staff (SVP (Senior Vice President) of Nursing, SVP of BH, medical providers) to establish, monitor, and evaluate MAT services to ensure effective delivery of care.
Serve as linkage to care contact for all new referrals for MAT services.
Provide initial phone or in-person screening for patients interested in MAT to determine appropriateness of treatment or identify appropriate programs outside of Zufall.
Meet with new patients receiving MAT to assess needs and assist with intake, coordination of care between Zufall Health clinical staff, patients, and their families.
Provide case management to these patients including linking patient to internal Zufall Health supports/programs, linkage to community resources, and advocacy
Track program participants and follow up regularly to ensure continued engagement in treatment
As part of QA/PI processes, track, and report on program data
Ensure best practices and efficiencies for effective delivery of MAT and in conjunction or separately of telehealth services, offering clinical care team workflow support and technical guidance as needed.
Work with patients and staff remotely or in person to ensure continuity of care and access to services, including some travel between sites.
Provide or arrange staff training on MAT, OUD (Opioid Use Disorder), and other related topics
Assess and identify gaps in program delivery and work with staff to resolve barriers to program delivery
Work with Zufall 340B, contract 340B and outside pharmacies to help facilitate access to medications, particularly injectables
Monitor 340B revenue stream as it relates to MAT services
Additional miscellaneous duties and responsibilities, as may be assigned from time to time by leadership
Knowledge, Skills, and Abilities
Ability to communicate clearly both in writing and verbally with students, health care team members, families, and staff
Ability to relate well to teens, adults, and families
Well versed in adult mental health and assessment
Learn and apply trauma-informed care principles within the scope of the position
Maintains calm demeanor and empathetic, patient-centered orientation even under stressful circumstances
Knowledge of Microsoft Office and use of electronic medical records
Education, Training and Experience
At least 1 year of prior, applicable experience is required.
At minimum, BA degree with commensurate experience in MAT and addiction services provision or other clinical degrees such as RN (Registered Nurse) or MSW
BLS Certification
Bilingual Spanish/English preferred
