  Job purpose

  Fair Haven Community Health Care (FHCHC) is a community health center whose success depends on community-based programming, outreach, and health-promotion efforts. The Medical Assistant for Outreach Programs is a key role that supports the clinical operations of two such initiatives: the Diabetes Prevention Program (DPP) and the Remote Patient Monitoring (RPM) Program. The role ensures that these programs successfully connect with patients, performs clinical duties in conjunction with the programs, and maintains data and inventories for the programs. 

  Duties and responsibilities

  Reporting to the Diabetes Prevention Program Nurse, the Medical Assistant for Outreach Programs will:

                (RPM Program Duties)

  Add new patients to (and maintain) the remote patient monitoring outreach tracker

  Perform 24-72 hours post-training phone calls to RPM-enrolled patients to confirm transmissions and troubleshoot

  Call patients with zero transmissions from Tableau report once a week, or as directed by Program Manager RN

  Provide tech support to patients as needed and with the help of RPM team

  Maintain RPM inventory and refill of supplies at all Fair Haven Community Health Care sites once every two weeks

  (DPP Program Duties)

  Phone outreach to patients with risk factors for diabetes to engage in Diabetes Prevention Program as directed by Program Manager RN

  Prepare patients in advance of seeing the clinician as appropriate based on the visit type, chief complaint, and clinician’s preference

  Perform tasks related to the visit that may include: documentation of the chief complaint, review of medications and allergies, vital signs, completing preventive health screenings based on the age and sex of the patient, perform lab testing and/or procedures with a clinician order or per standing orders

  Answer and document incoming patient calls and respond to patients under the direction of the Program Manager RN

  Scheduling at-risk patients for Oral Glucose Tolerance Tests

  Hands-on support of patient and workflow during twice monthly Oral Glucose Tolerance Test screenings/education sessions, which may occur on Saturdays

  (General Duties)

  Perform other necessary duties as required by FHCHC to achieve the goal of providing excellent primary care in a patient-centered medical home

  Participate in departmental meetings

  Adhere to all health, safety, and infection control protocols/policies

  Function as an effective and competent team member, demonstrating the ability to work effectively in a team environment

  Engage in performance improvement activities and act as an agent of positive change by looking for innovative ways to improve processes and advance clinical excellence.

  Qualifications

  High School Diploma or GED and successful completion of a Medical Assistant program (diploma) from an accredited school or be certified as a medical assistant with a minimum of three years’ experience. The successful applicant must m aintain medical assistant certification and FHCHC-specific medical assistant competencies. Basic Life Support Certification is required.

  Oral and written proficiency in English and oral bi-lingual fluency in Spanish required.

  The selected candidate will have a thorough knowledge of medical terminology and relevant procedures, excellent written, electronic and oral communication skills, above average critical thinking and problem solving skills and strong computer skills.

  Physical requirements

  Physical Demands: Requires walking, bending, sitting, standing, writing, reading, telephone use, data input into computer, pulling medical records,

  Mental Demands: Ability to cope with continual changing priorities under potentially stressful conditions

  Manual Dexterity Required:  Ability to use a keyboard, telephone.
