　　Newfane Rehab and Health Care Center is looking for an MDS LPN/RN to join our staff. This position requires a self-directed, responsible individual who will oversee all aspects of resident care and assure continued delivery of service.

　　$10,000 sign-on bonus

　　One year of experience is preferred.

　　Responsibilities and Duties * As a member of the team, this position requires critical thinking skills and the ability to demonstrate a level of proficiency in the management/oversight of their team. * Interaction with families, departments such as Therapy, Dietary and Social Work to ensure comprehensive person centered care is delivered is an essential part of the position * Sub-Acute Rehab as well as Long Term Care * 8 hour shifts * Current license in New York * Qualified candidates would be required to be flexible in their schedule * Compassionate individuals * Candidates with experience in dementia care, respiratory care, post operative care, wound care and end of life care will find satisfaction in working in this facility.
