　　Coordinates the interdisciplinary team in resident assessment anddevelopment of resident plan of care according to the ResidentAssessment Instrument (RAI) and state and federal requirements.

　　Job Responsibilities:

　　Assess residents in order to accurately complete the Minimum Data Setand Resident Assessment Protocol.Coordinate MDS and Care Plan Team

　　Implement and monitor individual care plans for residents based on MDSGuidelines.

　　Maintain a schedule of due dates for MDS and Care Plans including newadmissions and those with significant changes.

　　Submit the MDS to the appropriate agency.
