　　Job Description:

　　Supervised by a Registered Nurse, Practice Manager, or Physician, the LPN guides our patients through a seamless process to ensure that their care is of the highest quality and delivered in the most efficient manner. Directing the patients through the appointment, provisions of nursing services during care, and phone triage. Collaborates in care planning discussions, based on RN, MD or other Providers -determine urgency of care needs, triage of emergent care situations to the appropriate provider, and scheduling of acute care appointments to maximize patient care opportunities and unit efficiency. Maintains the flow of the Provider schedule during patient care sessions minimizing patient wait time whenever possible. Provides back up to the physicians and other Providers for patient care follow up needs within the scope of their practice.

　　Essential Job Functions: Provides direct patient care to patients in the age range of adolescence through advanced age, according to community standards for outpatient care, OB/GYN protocols/procedures, hospital standards, and professional nursing standards of practice

　　Salary Range:

　　$23.00 - $26.77 an hour

　　The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

　　Job Requirements:

　　Education: Fulfilled the educational requirements to be a licensed LPN in NYS.

　　Experience: 1 year of LPN experience preferred. Ambulatory outpatient women's health experience preferred

　　License/Certification Required: Current NYS LPN license and Health Care Provider CPR certification required

　　Skills: Must be a team player, be able to multi task and have basic computer literacy skills.
