Licensed Practical Nurse (LPN) - Medical/Surgical/Telemetry

　　PRN IV (Minimum 3 shifts/week, 4 weekends shifts/month, 3 holidays/year (Minimum 2 major holidays))

　　* Must have at least 12 months of hospital LPN experience. *

　　Position Summary:

　　As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice.

　　Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.

　　Performs plan of care interventions, including specimen collection and wound care. May administer medication. Documents patient care given.

　　Coordinates and incorporates the patient and family into the plan of care and delivery of care interventions, taking into consideration psychosocial/spiritual/cultural concerns.

　　Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.

　　Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.

　　Works as an advocate for the physical and emotional well-being of the patient.

　　Minimum Qualifications:

　　Current licensure as Licensed Practical Nurse from the Virginia State Board of Nursing. Can practice as an LPN per the Virginia Department of Health guidelines.

　　Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

　　Verbal ability to present information and ideas related to nursing theory and practice.

　　Ability to accept responsibility and accountability to perform delegated aspects of the nursing process.

　　Requires the cognitive ability and practical ability to provide nursing care appropriate to the patient population assigned, considering patient age and diagnosis with full implementation of the nursing process to include Assessment/Reassessment, Nursing Diagnosis, Planning, Implementation, and Evaluation.

　　AHA BLS certification required.

　　PALS certification, if applicable.
